Primary Rhinoplasty

The operations that improve the aesthetic appearance of the nose are called “Rhinoplasty” and the operations performed for health reasons are called “Septorinoplasty”.

Primary Rhinoplasty

The nose, which is the most aesthetically attractive structure of the face, has important duties such as heating, cleaning and humidifying the air we breathe in addition to the sense of smell.The operations that improve the aesthetic appearance of the nose are called “Rhinoplasty” and the operations performed for health reasons are called “Septorinoplasty”.

In aesthetic nose surgeries, it is not possible to achieve a perfect result in terms of correcting the appearance because each patient’s tissue responds differently to the surgery.Although the nose is a single organ, it is located in the midline of the face and it has a structure with two sides. A symmetrically similar appearance of the nose makes it difficult to achieve excellent success.

This surgery can change your physical appearance and increase your confidence. But before you decide to have surgery, you should think very well of your expectations and share it with your doctor.TThe most suitable candidates for rhinoplasty are people who do not expect a perfect result, but have the purpose of correcting the appearance of the nose.The success of the surgery depends on the surgeon’s knowledge, skills and experience as well as the patient’s age, general health, the structure of the nasal skin, the degree of nasal appearance, the tissues and some specific problems of the patient.

Sedation (sedative drugs) is done before surgery. Surgery is performed under general anesthesia.During surgery, the nasal skin is separated from the underlying cartilage and bone tissues and formed into the desired shape. The shaping process depends on the patient’s problem and the technique chosen by the surgeon.Generally, excess tissue pieces are removed with very precise cutters. The extracted tissues can sometimes be used to complement the deficiencies present in another part of the nose.Aesthetic nose surgeries can be performed for women between the ages of 16-17 and for men from the age of 17-18.

Revision Rhinoplasty

In rhinoplasty operation, even small errors can be seen as irregularities such as significant asymmetries, respiratory dysfunctions, protrusions on the nasal ridge and tip.

Nose revision surgery is most commonly performed as a result of the nose being reduced too much.Because too small nose causes narrowing of the air ducts and consequently breathing complaints. It also causes the nasal skin to be unable to adapt to the new structure, resulting in downward sagging of the nasal skin.In addition, asymmetries caused by different interventions in both parts of the nose are also common deformations. This and many other aesthetic and functional defects can be removed by secondary surgery.

  • Piezo Rhinoplasty 

A piezo rhinoplasty is simply the technical name for an ultrasonic rhinoplasty. This is a new form of rhinoplasty which avoids a hammer and chisel technique to reshape the nasal bones. It provides more precision and reduces the inadvertent trauma and soft tissue damage which can occur in traditional rhinoplasty techniques 

  • How does Piezo rhinoplasty work ?

This form of rhinoplasty avoids hammers and chisels which are a relatively crude from of instrumentation. Instead a piezotome is used to reshape and reduce the nasal bones. A piezotome is a specialized instrument which transmits ultrasonic energy to the bone without damaging any surrounding soft tissue. Since the energy is transmitted specifically to the bone there is no collateral damage which means soft tissue swelling and bruising is reduced. The piezotome device comes with various attachments shaped in the form of sculpting tools which are used to reshape the bone.

  • What is the advantage of Piezo rhinoplasty ?

This is a more gentle technique which affords the surgeon greater precision. There is less swelling and bruising and some patients find there is hardly any bruising at all. A hammer and chisel technique can result in some splintering of the bone and inadvertent fractures. This is avoided with a piezotome. It also allows for sculpting in a way which is not possible even with fine rasps. In some cases breaking of the nasal bones can even be avoided.

The technique also lends itself to the modern trend of preservation rhinoplasty which is a philosophy of reshaping rather than a destructive removal of tissue during rhinoplasty which is so common in old fashioned techniques. The piezotome can be used to reshape the bone without breaking the bone but also can be used for preservation of the nasal bridge.

Another important factor to consider is that a piezo rhinoplasty needs to be performed using an open approach and cannot be used with a closed or intranasal approach. This is because the instruments are not long enough and direct visualization of the bones is required for the piezo bone sculpting. However, the advantages of using the piezotome for the rhinoplasty outweigh the presence of a minimal scar which is imperceptible.

  • What is the recovery from a Piezo rhinoplasty ?

There is little or no pain following surgery. Also bruising is significantly reduced. A nasal splint is required in the usual way to keep the nasal bones in position during the initial healing period. This splint is removed approximately 1 week later.  At the time of splint removal the majority of patients will discover minimal or no bruising. The photo here shows an example of the lack of bruising visible at one week as soon as the nasal cast has been removed. 

However, it should be remembered that since a number of different structures are altered during the rhinoplasty it can still take approximately a year to see the final result after full healing. Unfortunately there is no fast track to the final result but certainly there are advantages in the early recovery period and in the precision of the technique which provides benefits in the quality of results obtained.

9- Since the nose would recover within 6 months, please ignore the comments of persons failing to have any knowledge on this operation

In the case of the deviation of the septum which is the name given to the compartment that separates both nostrils is called septoplasty 

Cartilage forms the anterior part of the septum and bone tissue forms the posterior part.If the cartilage and / or bones are displaced to one side of the nasal passage growth is observed across the side of the slope, called the concha.This structure has the function of heating and humidifying the air.Unilateral or bilateral disorders which are usually related to trauma can be resolved by septoplasty. These include nasal obstruction, unilateral discharge, nosebleeds, frequent recurrent attacks of sinusitis, headache, discharge from the nasal passages, snoring, and apnea in sleep.

In patients with septum deviation, mouth breathing must be performed instead of nose. In this disease, the heating, humidifying and cleaning functions of the air are disabled and the air goes directly into the lungs.Deviations also affect the sense of smell and thus taste. Fatigue and excessive drowsiness occur during the day due to snoring.Bad breath, dry mouth, dental diseases also affect the patient’s life negatively.

Surgery is usually performed under general anesthesia. After the operation performed by entering through the nostrils, no visible scar appears.Although swelling of the face and eyelids does not occur, conventional tampon is not applied and rarely one-day silicone tampon can be used

Tip Plasty

Tip plasty operation is mostly performed in patients with very large, very narrow, low or asymmetrical nostrils.

Very successful results can be obtained with this intervention in patients with problems such as low nasal tip, non-prominent nasal tip, discrete nasal tip, wider nasal wings, poor support of nasal wings, bulbous or square appearance of nasal tip.Tipplasty operation is an operation performed for patients who do not have any defect in the arch structure of the nose, and who feel the necessity of plastic surgery only on the tip and holes of the nose.This procedure is routinely performed within the classical rhinoplasty procedure. On the other hand, if there is only nasal tip deformity in the nose, this procedure can be performed alone.

Before the operation, the patient’s complaints and the reason for the operation are examined first. The simulation is then performed on face profiles drawn from different angles.Upon the expectation of the person, the most natural and functional form of operation is determined according to the face profile.

Otoplasty

Otoplasty is the operation performed against the ear deformity, which is commonly known as flap-ear.Flap-ear is the most common disorder, which is seen congenital or later.This disorder is due to the fact that the angle of the ear to the skull is greater than normal.This discomfort which makes the ear appear prominent can also be accompanied by the absence of the inner curves of the ear.

The aim of the operation is to normalize the angle of the ear with the head and at the same time to reconstruct these folds.Scoop ear surgeries are very diverse. Each doctor applies a separate technique. There are many techniques in the form of string flap ear surgery, open ear surgery. This operation is performed with general anesthesia in children and local anesthesia in adults. The operation takes about 1-1.5 hours. After the incision made behind the ear, permanent sutures are made to correct the folds and angle of the cartilage.

The patient returns home the same day after surgery. The edema and swelling are resolved within 2-3 weeks and it takes 2 months for the ear to take its final shape. Because the incisions are made from the back of the ear the scar is not visible.

The jaw tip  which is one of the most important structures forming the face profile such as the nose tip, should be proportionate and compatible with the other parts of the face.Mentoplasty is usually evaluated together with nasal aesthetics in Profiloplasty. In this way, the prosthesis placed on the tip of the jaw provides the nose, jaw and face proportion.

Before the operation, the profile of the patients is evaluated with the photograph taken and side cephalometric x-ray and the closure of the lower and upper jaw is examined.If there is a problem in this respect the patient should be consulted by the dentist about the need for orthodontic treatment and operation of the jaw bones.After this simple and safe operation, serious improvement and rejuvenation of facial aesthetics can be achieved.In jaw surgery with general anesthesia, the jaw prosthesis is inserted through the mouth or through a small incision under the jaw.

The sinuses, which moisturize the air we breathe and help to remove germs also lighten our head bones and play an important role in the characteristic formation of our voice.As a result of nasal obstruction, if the sinuses are not properly ventilated, bacteria or viruses will accumulate in them, which can cause bacterial sinusitis.Sinusitis may become chronic and surgical intervention may be necessary as a result of this increase especially in autumn and winter months.

Sore throat, dryness and crusting of the nose, halitosis, snoring, nasal congestion, loss of smell, constant fatigue, dry cough, voice change, nasal discharge and upper respiratory tract complaints lasting more than 3 weeks are the precursor of chronic sinusitis. In these cases, seeing the doctor is the most appropriate option.

Endoscopic examination can be used to diagnose intranasal causes before sinusitis surgery, which is a very sensitive operation. In acute sinusitis, patients can recover with antibiotics, nasal drops and antipyretics / analgesics in 10-14 days. In patients with chronic rhinosinusitis, endoscopic sinus surgery can be applied when there is no response to medical treatment lasting 1-2 months.Approximately 80 percent of chronic rhinosinusitis patients recover completely by surgery. The balloon synuplasty technique, which has been applied in recent years, provides early recovery with a lighter intervention. However, this technique can be applied to only 10 percent of patients.


Tonsillectomy

The tonsils which are located in the oral cavity on both sides of the tongue root and collecting germs that enter the body from outside, also provide the defense substances in the body.This function of the tonsils, which are more important in early childhood, especially when children recognize germs and where the immune system responds to germs, decreases with age.

In some children, around the age of 4 to 5, the tonsils grow excessively and become a source of infection due to frequent infection.Sometimes, because of their large size, they prevent breathing and eating, and cause nighttime respiratory arrest. In this and the following cases, tonsils should be removed by surgery.

  • Nasal flesh and tonsil growths that disrupt the jaw structure.

Tonsils, removed from the muscle and the cover under general anesthesia, which is opened with a special tool around the mouth.Then, bleeding control is performed and the operation is terminated. Classical techniques such as scissors, scalpel can be used for the removal of tonsils, as well as electrical incineration or similar technologies.


Ear Tube Surgery

If fluid accumulation in the ear does not respond to treatments, your doctor may need to perform surgical treatment to remove fluid from the middle ear, that is, insert a tube.

In patients with recurrent acute otitis media, middle ear fluids that do not heal themselves or with medical treatment, eardrum collapses, barotrauma during flight or diving, in patients with down syndrome, such as cleft palate or eustachian tube, the insertion of small tubes into a hole in the eardrum is called tubulation.Thanks to this procedure, the accumulation of fluid in the middle ear is prevented and both the hearing status of the child is improved and long-term problems related to this fluid accumulation are prevented.

In this process, microsurgical techniques are advanced and the liquid in the middle ear is pulled out of the hole through a vacuum in the scratched eardrum.Then, by means of special instruments, the tube is placed with one end on the outer ear and the other end on the middle ear. There can be no external changes to the person’s ear after this procedure. There is no need to make an incision in the skin in this operation performed under general anesthesia.

The ear tube usually remains in the eardrum for 6-12 months. It then falls into the ear canal spontaneously and the hole closes in the eardrum in which the tube is inserted.


Vocal Surgery

The lung transmits air to the larynx and produces air noise through the larynx vocal cords so that the breathing air can turn into sound and speech.This voice is accompanied by the oral cavity, teeth, tongue and sinuses. The hoarseness includes not only low sound, but also bifurcations, vibrations, and muffled sound.

The causes of hoarseness are generally inflammation of the larynx, nodules or polyps that may occur in vocal cords, paralysis of nerves that cause vocal cords to move, nasal discharge, reflux, laryngeal tumors, throat blows, diabetes or nervous system diseases and psychological causes.

The patient will sleep for approximately 20 minutes under general anesthesia. The surgeon will take the nasal flesh through your child’s mouth. There is no need to make an incision in the skin for nasal surgery.Your child will be awake in the waking room.
