The Procedure For Face Lift Surgery

There are three main types of Face Lift surgery; Traditional, Minimal-Access Cranial Suspension (MACS) and Keyhole Surgery. Your surgeon will advise you on which method is most suitable and which will produce the best results – details of each procedure can be found below. For many people, combining a Face Lift with Neck Lift surgery ensures that the overall appearance is rejuvenated and patients who undergo this combination of procedures have reported looking 10 years younger.

 

Traditional Face Lift

Traditional Face Lift will involve your surgeon making a surgical incision along your hairline, down past the front of your ears and into the hairline behind the ears. Because of the careful positioning of the incision, your scarring will be hidden within the hair and natural creases of the face and it will fade quickly until it is barely visible. Once the incision has been made the surgeon will very carefully separate the skin from the underlying tissue, before repositioning excess fat and tightening facial muscles. The skin will then be lifted and stitched into place along the original incision and any excess skin removed.

Once the surgery is complete, your face will be wrapped in bandages to prevent infection and to support your face while the skin heals, but these will usually be removed the day before you are allowed to go home from hospital. Drains will also be stitched into place behind the ears to allow excess blood and fluids to drain from the area – these are sometimes left in for the first few days but this is decided on a case-by-case basis.

Minimal-Access Cranial Suspension (MACS)

The main difference between this type of Face Lift surgery and the traditional method is that the incisions made are smaller and the tightening of facial muscles and removal of excess fat is done through these small surgical incisions.

It involves lifting the brow itself and tightening the muscles which have been stretched over time. It works to tighten any loose skin in brow area and removes any excess fatty tissue.

The Procedure For Brow Lift Surgery

The Brow Lift operation is performed under general anaesthesia and takes between one and two hours to complete depending on the technique used. There are two types of Brow Lift surgery (outlined below) and the one chosen by the surgeon for you will depend on your skin type, the condition of your skin and where your hairline is.

Open Brow Lift

If the Open Brow Lift is the most appropriate for you, the surgeon will make an incision from one ear, along or slightly back from the hairline to your other ear. Please note that for bald patients or those with a receding hair line, the incision can usually be hidden in a natural forehead crease. Once the incision has been made the surgeon will carefully separate the skin on the upper part of the face from the underlying tissue before working to reduce or tighten the muscles. The skin is then stretched and stitched or clipped in place, within any excess skin removed.

Endoscopic Brow Lift

This brow lift procedure sees the surgeon making three to five small incisions in various locations on the scalp through which a thin plastic tube with a tiny camera at one end is inserted. The images gathered from the camera are relayed live to a screen in the theatre and this allows the surgeon to operate on the muscles and fatty tissue behind the hairline. The eyebrows are lifted and held in place with stitches, or temporary screws behind the hairline and the incisions are either stitched or clipped.

What happens during the surgery?

The Day Of Your Brow Lift Surgery

If you are a smoker, or take aspirin or any other medication, your Cosmetic Surgeon will wish to discuss this with you at your consultation. Your Cosmetic Surgeon will also advise you when you are to discontinue and/or resume any physical activities and medication that may interfere with the operation.

The Cosmetic Surgeon will visit you in your room. The Cosmetic Surgeon will also at this time obtain your final consent, and answer any last minute questions you may have.

A Brow Lift procedure requires an overnight stay in hospital and the operation is performed under general anaesthesia, the operation normally takes one – two hours to complete. The technique used depends on your facial structure and skin condition and also on the hairline, as this determines where the incisions will be made.

Going Home After Your Brow Lift Surgery

The Cosmetic Surgeon will advise you in relation to how long to stay off work – usually between one and two weeks. You will need to sleep upright for one week after your brow lift surgery to reduce the swelling. You should avoid styling products and wash your hair daily with baby shampoo and dry suture areas thoroughly.

It is very important that you attend the follow up post Cosmetic Surgery appointment which has been made for you, to care for your wound and see your Surgeon post-operatively to make sure that you are healing well. You will have your own personal Nurse Counsellor both before and after your procedure to support you and answer any questions you may have.

Recovery From Your Brow Lift Surgery

For both types of brow lift surgery, your stitches will be removed approximately 10 days after the procedure and you will be able to return to work within two weeks. Any scarring will be minimal and will be hidden under the hairline or in the natural forehead creases – they will also fade within three to six months.

What are the things to consider?

The Day of Your Neck Lift Surgery

The Cosmetic Surgeon will visit you in your room to obtain your final consent and mark the face with a pen in preparation for your surgery. If you have any final questions it is important that you ask your Cosmetic Surgeon at this time.

A Neck Lift involves the skillful and careful removal of excess facial skin from the neck, chin and cheek areas, under a general Anaesthesia. When you wake up from your surgery, you will have dressings on the next and possibly drainage tubes in place.

Painkillers will be offered if required, however it is not regarded as a painful procedure, more uncomfortable due to the ‘tight’ feeling. You can expect to be in hospital for 1 or 2 days, any drainage tubes will be removed before you go home.

Going Home After Your Neck Lift

It is advisable to rest with your head propped up in the early days and avoid bending, lifting or sudden movements of the head. You will be advised on when you can wash your hair and this will help to make the removal of the stitches easier. A scarf will easily hide any bruising and swelling. Although make up can be used, it must be well away from the stitches.

It is very important that you attend the appointments, which have been made for you to care for your wound and see your Cosmetic Surgeon. It will be possible to return to work once the stitches have been removed and the bruising and swelling have settled, approximately 2 – 3 weeks.

You will have your own personal Nurse Counsellor both before and after your procedure to support you and answer any questions you may have.

Recovery From Neck Lift Surgery

As with all cosmetic surgery procedures, the recovery time following Neck Lift surgery will vary depending on the patient. However, most patients will be required to stay in hospital overnight and be advised to take between one and two weeks off work to recover fully. There will be localised swelling and bruising but this should ease within 14 days of the operation to reveal younger, smoother looking skin on the neck. Any scars left by the incisions will fade quickly and will be barely noticeable due to their discreet locations behind the ears and in the hairline on the neck.

The Procedure For Eye Bag Removal

Eye bag removal surgery, also known as Blepharoplasty, is a straightforward procedure which usually takes between one and three hours to complete and is often carried out as a dUpper Eyelid Surgeryay case procedure. The length of time you are in theatre for and your recovery time will depend on the specific surgery you decide to have – this could be surgery to the upper lids, lower lids, or together.

Upper Eyelid Surgery

For some patients, upper eyelid surgery only requires the use of a local anaesthesia and a mild sedative, meaning the recovery time should be quicker than if a general anaesthesia is used. Your surgeon will begin by making small incisions along the natural crease of the upper eye lid (approximately 1cm above the lash line). He or she will then very carefully separate the skin from the underlying tissue before removing or repositioning the fatty tissue and tightening the muscles. Before stitching or gluing the incision using surgical glue, the skin will be stretched to ensure that the end result is smooth and free from lines and wrinkles.

Lower Eyelid Surgery

If you opt for lower eyelid surgery, or a combination of upper and lower eyelid surgery, this will almost always be performed under general anaesthesia. Your surgeon will make an incision along the lower lash line before removing any excess fat (often known as eye bags) and stretching the skin to ensure a smooth finish. Once the incision has been stitched, any excess skin will be removed and you will be left with smoother, younger looking skin underneath the eyes.

What happens during the surgery?

You may be advised to undergo an eye test if you have any concerns about health issues in your eyes. You should also purchase a gel pack for when you get home from the hospital to act as an ‘ice pack’ to reduce swelling and bruising.

Immediately after your surgery you can expect to have ice packs over your eyes to keep bruising and swelling to a minimum. Painkillers will be available should you require them and eye ointment may be given to you 

Going home after Eye Bag Removal surgery

  • Your Cosmetic Surgeon will advise you in relation to how long to stay off work
  • Usually between one and two weeks depending on the extent of the surgery
  • You will need to rest and avoid bending down
  • If eye ointment has been given to you, you will need to continue using this as recommended by your Cosmetic Surgeon.
  • Continue to use your ‘ice pack’ to relieve bruising and keep the scars dry and clean until stitches are removed
  • A pair of dark glasses will help to hide the bruising and swelling during this recovery period.
  • You will need to rest with your head elevated on pillows or cushions for 4 nights after your surgery.
  • You must avoid rubbing or towelling your eyes.
  • It is very important that you attend the follow up post surgery appointment which has been made for you, to care for your wound and see your Cosmetic Surgeon post-operatively to make sure that you are healing well.
  • You will have your own personal Nurse Counsellor both before and after your procedure to support you and answer any questions you may have.

Recovery From Eye Bag Removal

The incisions made by your surgeon will be very carefully positioned along the lash line and in the natural creases of the eye to ensure that any minimal scarring will be hidden. It is completely normal to experience swelling and bruising to the area but this tends to settle within the first few days of the recovery period and any pink scars will quickly fade to faint white lines. Your stitches will usually be removed between three and seven days after surgery and you will be advised not to return to work for a week after the operation.

Blepharoplasty is a type of cosmetic surgery that involves removing excess skin or fat from the eyelids (top, bottom or both).

How Long Does Blepharoplasty Last?

Results from your Blepharoplasty surgery should be long-lasting for around 10 years. However, this will depend on several factors such as the ageing process, medical conditions and your genetics.

How Do You Get Rid Of Bags Under Eyes?

Surgery to get rid of bags under eyes involves an incision being made in the lower eyelid area, removing or repositioning fat from the bags and also a small amount of skin in some cases. In addition, the muscles and tendon of the eyelid may also be supported before the incision is closed.

Is Eyelid Surgery Painful?

Eyelid surgery can be performed under either local or general anaesthesia. Immediately after surgery your surgeon will apply antibiotic ointment to your eye, which may cause you to have blurred vision, and experiencing dry eyes, numbness and surface irritation is also normal for a short time after the procedure. If you do experience any pain or discomfort after your surgery, this can be relieved by taking a non-prescription painkiller such as paracetamol.

THE DAY OF YOUR CHIN IMPLANT SURGERY

The Cosmetic Surgeon will visit you in your room and may mark the chin with a pen in preparation for your implant surgery. Your Cosmetic Surgeon will also obtain your final consent, and answer any last minute questions you may have. The implant is positioned beneath the skin through an incision made from inside the mouth or below the chin and sutured with dissolvable sutures.

GOING HOME AFTER YOUR SURGERY

Your Cosmetic Surgeon will advise you in relation to how long to stay off work – usually 1 week. Your wounds should be checked after 1 week.

If the incision was made inside the mouth, take soft fluids only through a straw until more comfortable. You should also avoid hot/spicy foods and use anti-bacterial mouth washes frequently

It is very important that you attend the follow up post Cosmetic Surgery appointment which has been made for you, to care for your wound and see your Cosmetic Surgeon post-operatively to make sure that you are healing well.

Ear Reshaping (Otoplasty) surgery usually takes between two and three hours to complete while the patient is under a general Anesthesia. It is a straightforward procedure which involves the experienced surgeon making small and discreet surgical incisions behind the ears and within the ears to ensure that there is no visible scarring as a result of the procedure. What happens during the surgery?

The Procedure

Before your Ear Reshaping surgery we would recommend that you wash your hair the evening before surgery. The bandage will be on for 7-10 days continuously and during this time you will not be able to wash your hair.

If you are a smoker, or take aspirin or any other medication, your Cosmetic Surgeon will wish to discuss this with you at your consultation. Your Cosmetic Surgeon will advise you when you are to discontinue and/or resume these activities.

The Day Of Your Ear Reshaping Surgery

The Cosmetic Surgeon will visit you in your room prior to surgery and may mark the ears with a pen in preparation for your Ear Reshaping surgery. Your Cosmetic Surgeon will also obtain your final consent, and answer any last minute questions you may have.

Going Home After Ear Reshaping Surgery

  • There may be some residual bruising after your Ear Reshaping surgery, but the scars will now be well hidden behind the ears.
  • You should avoid sleeping on your sides in order to reduce ear pressure.
  • Once the bandages and any sutures are removed, we recommend regular hair washes/showers to clean behind the ear – avoiding rubbing, as the ear will remain tender for a while.
  • For further 7-10 nights we recommend you wear a sports headband to protect the ears whilst in bed.
  • It is very important that you attend the follow up post surgery appointment which has been made for you, to care for your wound and see your Cosmetic Surgeon post-operatively to make sure that you are healing well.

COSMETIC FACIAL INJECTIONS

Fillers to treat facial wrinkles

Dermal Fillers

Many methods and substances have been used to treat facial wrinkles and to conceal the effects of aging on the face. In addition to formal surgical procedures, laser treatment and botulinum toxin there are a large number of fillers available which can be injected as an outpatient. These are the subject of this information sheet.

Types: Dermal fillers may be biocompatible or synthetic and are marketed alone or in combination.

Biocompatible fillers have a perceived safety record and are resorbable. However the period of benefit is of limited duration normally lasting about 6 months. Synthetic fillers are permanent but are associated with rejection, migration and granuloma formation and are unable to change with the ageing face. In the case of an adverse reaction removal is almost impossible to achieve. There are few indications in current practice for the injection of synthetic fillers to the correct lines and effects of ageing. They are included for reference rather than recommendation.

Biocompatible Fillers

Ideally the effects of a filler should last about 12 months. This allows reasonable respite from the injections and also enables accommodation of changes in the face which may occur with continued ageing.

Collagen

Collagen implants are prepared from three sources: bovine, human or autologous. Collagen synthesis may also be increased by fibroblast stimulation or injection of cultured cells.

Bovine Preparations – This comes in a highly purified or monomolecular form in different concentrations and mixed with local anaesthetic. A test dose is required as approximately 3% of patients demonstrate sensitivity and another 1% will develop sensitivity with treatment. In most patients it will be completely resorbed within 3 to 6 months.

Human Preparations – These are a preparation of human collagen, elastic fibres and other dermal components from screened donors. No pre-treatment skin testing is necessary and the effects last from 3 to 12 months.

Autologous Preparations – These require skin harvest, which is processed into a collagen suspension for injection and requires 2-3 treatments over a 3 month period. This is reported to provide greater than 75% correction for more than a year.

Injectable Fibroblasts can be cultured from a small skin biopsy and following a test dose at one month a series of 3 injections is given at 2 week intervals. Gradual improvement occurs over 6 months and repeat injections can be prepared as required from the original biopsy.

Polylatic acid stimulates fibroblast production. The effect takes 4 to 6 weeks to achieve but can last for years. This should not be injected superficially as obvious fibrous nodules can result which may be difficult to treat.

Hyaluronic Acid

  • Hyaluronic acid preparations (hyaluronan) are obtained by bacterial synthesis or from an avian source and are cross linked to varying degrees to maximise viscoelasticity and persistence. They last between 6 and 9 months and although well tolerated there is a 1% incidence of adverse reactions including granulomatous inflammation and sterile abscesses.

Synthetic Permanent Dermal Fillers

Types Available

Methylmethacrylate has been used for many years as bone cement for joint replacement surgery. However in the skin redness, inflammation and infection may occur. One preparation utilises Methylmethacrylate microspheres suspended in bovine collagen whilst an alternative suspends them in Hyaluronic acid. These are intended to permit even distribution of the microspheres prior to absorption of the collagen or Hyaluronic acid. A series of injections are required to achieve the final result and allergic reactions to collagen may occur.

Polyacrylamide Gel is used alone or in combination with polyvinyl microspheres. The injected volume is reported to diminish by one third post injection and then build up again over the next 2 months. The augmentation is supposed to be permanent but additional injections may be required.

This augmentation tends to stay soft but Polyacrylamide gel can act as a reservoir for bacteria and has been associated with a significant delayed infection rate.

Silicone Injections are banned in most countries but are still used in France and Italy. Although giving apparently excellent initial results, migration and inflammation are almost inevitable and the surgery then required is likely to result in much greater aesthetic deformity than that leading to the initial treatment.
